Greenville, S.C. – September 17, 2026 -- Senior Helpers Greenville has become the first private duty in-home care provider in the United States to earn Age-Friendly Care at Home certification from the Community Health Accreditation Partners (CHAP), the locally owned franchise confirmed August 27, 2026.

CHAP certification validates a 4Ms clinical framework across daily operations

The certification requires providers to embed the 4Ms Framework -- What Matters, Medication, Mentation, and Mobility -- into every client interaction, a research-backed model designed to reduce hospitalization risk and improve outcomes for aging-in-place seniors. Teresa Harbour, COO of CHAP, credited the location's prior deployment of clinical tools, including its LIFE Profile hospitalization-risk assessment and Senior Gems dementia care program, as groundwork that made the certification a natural extension of existing practice.

Three proprietary programs underpin the compliance framework ahead of certification

Senior Helpers Greenville had already built its care model around three internal systems prior to certification: the LIFE Profile data tool for hospitalization-risk scoring, the Senior Gems program addressing care needs from normal aging through late-stage dementia, and a Center of Excellence training facility that simulates in-home conditions for caregiver skill assessment. CMS-approved accreditor extends its home-care standards to private duty operators

CHAP, a nonprofit accrediting body founded in 1965 and approved by the Centers for Medicare & Medicaid Services (CMS), has historically certified home health, hospice, and home medical equipment providers against Medicare Conditions of Participation. Its extension of the Age-Friendly Care at Home certification into the private duty segment marks a new accreditation category for non-Medicare-reimbursed home care operators, positioning the credential as a differentiator in a market where quality benchmarks for private duty care have been historically fragmented.
