San Francisco Health Plan Hits Record 4.5-Star NCQA Rating

San Francisco – September 18, 2026 -- San Francisco Health Plan (SFHP) has earned a 4.5-star rating in the 2026 NCQA Health Plan Ratings, the highest score in its history, as the Medi-Cal insurer covers more than 172,000 residents.

NCQA places SFHP among the nation's top-rated health plans

NCQA classifies plans scoring 4.5 or 5 stars as among the highest-rated in the country, basing the evaluation on clinical quality, member experience, and accreditation status. SFHP, created by the City and County of San Francisco, is the city's leading Medi-Cal provider, offering preventive care, specialty care, hospitalization, prescription drugs, and family planning services.

Milestone lands as SFHP approaches its 30th anniversary

The rating arrives as SFHP prepares to mark three decades of serving San Francisco's low-income and vulnerable populations. CEO Yolanda R. Richardson credited partnerships with providers and community organizations across the city for enabling the achievement.

Executives tie the score to preventive care and equity gains

Chief Medical Officer Steve O'Brien, MD, said the 4.5-star rating reflects sustained work on member outcomes and provider collaboration. SFHP attributed the result to multi-year efforts to strengthen preventive care, improve member experience, and advance health equity, with contributions from its Member Advisory Committee, Quality Improvement and Health Equity Committee, and Governing Board.
