Dallas – October 07, 2026 -- Patients who completed Annual Wellness Visits through Primary Partners Alliance's (PPA) population health strategy generated nearly $70 lower per-member-per-month healthcare costs than those who did not, according to a one-year analysis of the organization's Medicare Shared Savings Program and ACO REACH population.
AWV completion drove measurable cost and utilization reductions across PPA's network
The analysis found per-member-per-month costs of $947 for patients who completed an AWV versus $1,014 for those who did not. Readmission rates were 44% lower among AWV completers, at 9% compared with 16% for non-completers. Emergency department utilization also fell, registering 272 visits per 1,000 patients versus 286 per 1,000 for the comparison group.
HCC recapture rates rose to 70% among engaged patients
Hierarchical Condition Category recapture reached 70% for patients who completed an AWV, compared with 57% for those who did not, pointing to stronger diagnostic documentation and risk-adjustment accuracy for the engaged cohort.
PPA used Lightbeam's platform to prioritize outreach across 251 providers
PPA deployed Lightbeam Health Solutions' AI-enabled platform to identify patient risk, flag care gaps, prioritize outreach, and give care teams visibility into performance across its network of 251 providers. Alliance for Community Health, a nonprofit partner, supported the initiative through population health management assistance.
"Our goal is to give independent practices the visibility and support they need to succeed in value-based care while keeping the focus where it belongs—on the patient," said Lisa Bloder, Executive Director of Population Health at Primary Partners of Florida.
PPA's broader network spans 900 physicians and 750,000 patients in Central Florida
Primary Partners Alliance is a physician-owned and physician-governed organization bringing together more than 900 independent primary care physicians and specialists across 34 counties in Central Florida, representing approximately 750,000 patients. The AWV-based engagement model serves as an entry point for ongoing care management within PPA's integrated ACO, IPA and CIN structure.
Melissa Tyler, Vice President of Advisory Services at Lightbeam Health Solutions, said the results reflect the performance value-based care organizations are targeting when population health insight extends beyond a single patient encounter.
