AI Agents Recover $7M in Dental A/R as DSOs Fight No-Shows

Texas – October 29, 2026 -- 42 North Dental recovered more than $7 million in patient accounts receivable in seven months using IntelePeer's AI agents, including $1.8 million from balances older than 180 days, according to the vendor's customer data. The figures come as dental support organizations (DSOs) with 20 to 200 locations face no-show rates of 30% to 40% and a front-desk hiring shortage.

No-shows and unanswered calls drain production before patients reach the chair

IntelePeer identifies no-show rates of 30% to 40% for new-patient appointments as the leading driver of lost production in multi-site dental groups. It also reports that 85% of patients who cannot reach a practice do not call back and go to a competitor. Calls go unanswered at peak hours, during lunch and whenever front-desk staff turn over.

Jefferson Dental, with 73 Texas locations, faced a 40% no-show rate and call volume its staff could not absorb. It deployed 25 SmartAgents under the name Sofia, a bilingual AI receptionist available 24/7 for scheduling, reminders and patient intake. The group booked $210,000 or more in appointments through AI outreach alone in six months, with fewer no-shows and a lighter front-desk call load.

Fragmented point solutions leave operations leaders without performance data

Many DSOs have added separate scheduling, reminder and collections tools that do not share data. IntelePeer says some of its customers ran three or more patient communication vendors before moving to a single platform. Consolidation gave operations teams clearer location-level reporting, so they no longer have to reconcile four disconnected systems to learn the monthly no-show rate.

Collections odds fall 20% to 30% once accounts pass 90 days

IntelePeer says the probability of collecting drops 20% to 30% once an account reaches 90 days in A/R. Its AI collections agents work every account, from current balances to 180 days or more, and negotiate payment plans during the call.

Another top-10 DSO recovered more than $20 million in outstanding patient A/R, with about one-third tied to balances over 120 days old. 42 North Dental CFO Ajay Chadha attributed the $7 million-plus in collections to the seven months since going live.

Benevis cuts Q4 headcount needs by about 50% across 120 locations

The ADA Health Policy Institute reported in Q3 2024 that 91.7% of dental practices find recruiting front-desk staff very or extremely challenging. IntelePeer argues that staffing models that work at 20 locations break at 80.

Benevis, with 120 locations, reached break-even ROI within six months of deploying SmartAgent. It also cut Q4 headcount needs by approximately 50%. IntelePeer says ROI stems from incremental appointment revenue, staffing cost avoidance and A/R recovery.

SmartAgent writes directly into Dentrix, Eaglesoft and Open Dental

According to IntelePeer, SmartAgent integrates with Dentrix, Eaglesoft and Open Dental without replacing existing infrastructure. It reads and writes appointment data directly in the practice management system. The company says connecting voice, SMS, digital channels and live staff interactions into one operating picture lets leadership see where patients drop out of the funnel and which locations lag on scheduling conversion. For acquisitive platforms, new locations come online with reporting and performance benchmarks already in place.
