Upstream Rehabilitation Renews Raintree EMR Deal Across 1,200 Clinics

CHANDLER – October 29, 2026 -- Upstream Rehabilitation, the largest dedicated outpatient physical therapy provider in the United States, has renewed its enterprise agreement with EMR vendor Raintree, covering more than 1,200 clinics across 28 states.

ScribeIQ now supports more than 40% of Upstream's evaluations

Upstream has used ScribeIQ, Raintree's ambient AI scribe built for rehab therapy, to document more than 40% of the evaluations it records in Raintree. The stated aims are more complete documentation, more accurate coding and more clinician time with patients.

Raintree CEO Nick Hedges said Upstream has seen the AI deliver additional revenue per visit and time back for clinicians. The source provides no figures for either.

Agreement opens four additional modules for Upstream to evaluate

The renewal includes access to Raintree's IQ platform, including ScribeIQ, NoteIQ and SchedulerIQ. Upstream can also evaluate and adopt the following over time:

  • Pathways: referral management linking clinics with hospitals and health systems through secure data exchange.
  • Plan of Care DocuSign integration: digital signatures from referring physicians on plans of care.
  • Touchpoints: patient self-scheduling, digital intake, kiosk check-in and payment collection.
  • Gateways: a self-service API hub for Upstream's IT team to connect partners and systems securely.
Both CEOs stress continuity after years of growth

Upstream CEO Anthony Mollica said Raintree has supported the company through significant growth and that "continuity matters for our clinicians and patients." He said the two companies are focused on easing documentation and returning time to patient care.

Raintree reports 50 million visits a year across 8,500 clinics

Raintree says its platform powers more than 50 million patient visits a year across more than 8,500 clinics nationwide. Upstream's 1,200-plus clinics are therefore a sizeable share of the vendor's footprint, though the source gives no contract value or term length.

In July, Raintree acquired voice AI developer Spike Technologies to expand its AI capabilities in revenue cycle management and patient engagement.
