Only 6% of Firms Scale AI; Healthcare Pilots Stall on Architecture

GLOBAL – October 27, 2026 -- Only about 6% of organizations have scaled AI to meaningful financial impact, leaving 94% stuck between experimentation and abandonment, according to McKinsey's 2024 State of AI survey cited by IntelePeer in a new healthcare analysis.

The vendor calls the gap "Pilot Purgatory": an AI voice agent handles a scheduling demo flawlessly, a pilot is approved, and within six months it is shelved. IntelePeer argues the fix is architecture rather than a better algorithm.

Human review of every AI action turns automation into overhead

Greater AI autonomy demands more evidence of safe, predictable behavior, a tension IntelePeer labels the Trust Paradox. It is most acute in regulated healthcare, where wrong actions can be non-compliant or financially damaging. Requiring a human to audit every output creates what the company calls "digital overhead", a costlier version of the existing process.

Four layers separate production systems from stalled pilots

IntelePeer identifies four layers that must hold under production load:

  • Reliability: Large language models generate the most likely response, not the verified correct one. Production systems must bound what the model knows, does and says, and how it fails.
  • Real-time performance: On voice calls, gaps between conversational turns often run just a few hundred milliseconds, so a two-second pause reads as confusion. Systems routing audio through multiple vendors accumulate latency at each hop.
  • Integration: Read-only access to appointment data creates deflection. Write-back to practice management systems creates capacity.
  • Governance: Structured records of who the AI spoke to, what it said and which actions it took make the other three layers defensible under audit.
Thin LLM wrappers talk but cannot execute

IntelePeer says lightweight products built as user interfaces over commodity models struggle in enterprise settings. They can deliver a conversational experience but not completed work. The company expects early builders of a full stack to gain a compounding advantage, since each interaction generates data that improves speed and reliability.

Mark Langanki, Chief AI Officer at IntelePeer, said that taking too long to do too much in the first step is usually what causes fatigue and cancelled pilots, adding: "Not moving to production is the worst outcome as the value is never realized."

Vendor claims 12–18 months in pilot versus weeks for purpose-built platforms

IntelePeer states that organizations retrofitting governance, integration and reliability onto a demo-grade system often spend 12–18 months in pilot. Those starting on a purpose-built platform typically go live within weeks, the company says. These timelines are IntelePeer's own and are not independently verified.

The company's SmartAgent platform ties responses to grounded knowledge bases and runs on a carrier-grade voice architecture. It reads from and writes back to practice management systems, and uses SmartAnalytics for interaction visibility and outcome tracking.
