Equip Launches Transition Track to Cut Eating Disorder Readmissions

San Diego – October 07, 2026 -- Equip, a virtual eating disorder treatment provider, has launched Transition Track, a new care protocol designed to move medically stable patients ages 6 to 24 directly from inpatient hospitalization to home-based recovery without stopping at residential, PHP or IOP facilities.

Protocol targets the highest-risk window after hospital discharge

Transition Track is built on Family-Based Treatment (FBT), combining frequent therapy sessions, daily virtual meal supervision and caregiver coaching during the first weeks post-discharge, then continuing with the same multidisciplinary care team for a full 12 months. The approach aligns with guidance from the American Psychiatric Association, the Society for Adolescent Health and Medicine, and the American Academy of Pediatrics, all of which recommend family-supported outpatient care as the first-line treatment for medically stable patients.

Equip reports outcomes from nearly 6,000 patients treated over two years

The company says more than 90% of medically stable patients with severe eating disorders avoid being discharged to higher levels of care while in its program. Three in four patients reach minimal or subclinical levels of eating disorder, anxiety and depression symptoms within six months, and eight in 10 patients and families report high treatment satisfaction, according to Equip.

Care model front-loads clinical intensity before easing into maintenance

The protocol doubles standard FBT session frequency in its initial phase, pairs each family with a caregiver coach who has lived experience, and unifies a therapist, registered dietitian, medical provider and coaches under one coordinated plan. Patients practice recovery skills during actual family meals, school routines and social events rather than in a clinical setting.

"Having gone through recovery myself, I know firsthand how hard it is to take the skills you learn in a hospital and apply them to real life at home, which is why relapse happens so often," said Kristina Saffran, CEO and Co-Founder of Equip.

Equip operates nationwide with insurer and Medicaid partnerships

The company runs in all 50 states and holds partnerships with most major health insurance plans and a growing number of Medicaid plans, positioning Transition Track as a payer-facing alternative to costlier inpatient, residential and PHP placements for eating disorder care.
